File VA Form 10-8678 with the prosthetic representative at the local VA medical center by the annual deadline and provide requested device or medication evidence.

Cost and fees
No application fee. Approval is annual and deadline-dependent; ordinary clothing costs beyond the allowance remain the Veteran's responsibility.
