Requirements
The state agency determines whether the applicant has a physical or mental impairment that creates a substantial impediment to employment and requires vocational rehabilitation services to achieve an employment outcome. Service priority, financial participation, comparable-benefit, documentation, residency, and plan rules vary by state and service.

Find the designated state vocational rehabilitation agency in the federal state-award directory, request an application, complete eligibility assessment, and work with a counselor on an Individualized Plan for Employment before obtaining planned services.
Cost basis
Variable by provider or eligibility. The user's out-of-pocket cost cannot be represented by one fixed amount because it depends on provider policy, income, household, geography, selected service, coverage group, subsidy amount, or the underlying program. It may be free, reduced, sliding-scale, or partly paid.
Cost and fees
Application, eligibility assessment, counseling, and certain core services are not conditioned on ability to pay.
