Requirements
The person must apply at a currently obligated facility and meet its income and service rules. Coverage is not automatic and generally does not cover private physician bills.
How to activate
Find an obligated facility, request its individual notice and application from admissions or the business office, and apply before or after care.
Cost basis
Variable by provider or eligibility. The user's out-of-pocket cost cannot be represented by one fixed amount because it depends on provider policy, income, household, geography, selected service, coverage group, subsidy amount, or the underlying program. It may be free, reduced, sliding-scale, or partly paid.
Cost and fees
Eligible facility services can be free or reduced-cost; private doctors and noncovered services may bill separately.
